They are often the first port of call for individuals who are struggling with eating or weight. Unintentional weight loss is a red flag for underlying pathology e.g. cancer and it is important that alongside referral for further investigation, that the unintentional weight loss which may result in malnutrition is also treated. As GPs regularly see a number of patients who are at high risk of malnutrition they can play a central role in the identification, assessment and treatment of malnutrition in the following ways:
Consider incorporating nutrition screening: BAPEN MALNUTRITION UNIVERSAL SCREENING TOOL ‘MUST’ into pathways of care for members of the healthcare team to use particularly for high-risk groups such as the elderly and those with long term conditions or when there is clinical concern:
DIETARY ADVICE LEAFLETS are available to provide further ideas on managing the diet-related problems/symptoms. Local advice sheets may also be available.
It is important that patients understand the importance of adopting such dietary advice particularly if they have historically followed healthy eating plans and patterns, some discussion regarding following a more calorific diet may need to be had. ADVICE ON FOOD FORTIFICATION should be given. Refer those with eating and drinking difficulties on to other multidisciplinary team members such as Dietitians and Speech and Language Therapists
PRESCRIBE ORAL NUTRITIONAL SUPPLEMENTS (ONS) for those identified at high risk. Consider if protocol dictates that patient should be referred to a Dietitian for assessment prior to prescribing ONS whether waiting times for the referral will seriously delay intervention and the consequences of this delay on the patients’ health status. If so is there another member of the MDT (GP/Practice Pharmacist) who could be considered to prescribe ONS in the interim to prevent unnecessary deterioration while awaiting dietetic input. Discuss goals of the intervention with the patient, example of goals might include:

Online training
Speak to the local dietetic department about the initiation of training sessions for staff on malnutrition. Also consider online training tools, such as the BAPEN VIRTUAL LEARNING ENVIRONMENT
